The Partnership for Strong Communities is concluding its 2015 IForum series on Thursday, June 25th with Health Care, Housing, & Homelessness: Integrating Systems. Come learn how health care and housing providers can work together to ensure people experiencing homelessness  living with physical and/or mental health disabilities get the care they need at much less expense.

Innovative models in Connecticut are providing collaborative care, decreased public costs, and improved well-being. This forum offers the opportunity to discuss progress and expansion of current efforts.

  • Role of hospitals in identifying frequent visitors/super utilizers experiencing homelessness

  • Lessons learned from Community Care Teams

  • How to identify and house high-cost high-need Medicaid beneficiaries experiencing homelessness through the Social Innovation Fund (SIF)

  • Success of identifying and housing frequent users of the criminal justice system experiencing homelessness through the Frequent Users Systems Engagement (FUSE) program

  • Potential of respite programs to improve outcomes for those experiencing homelessness who are living with health conditions
